ADMISSIONS
The ACJVS Workforce Development Office staff of trained professionals will help you through the admissions process, from course registration through the completion of financial aid applications, and any testing that may be required

REGISTRATION
Admission is open to anyone 16 years of age and older.  Registration is required for all adult courses, and must be accompanied by a $25 fee.  This fee is non-refundable if the class runs.

Once you have decided on a course, you may register in person, by mail, by phone, by fax, or by visiting our on-line enrollment page.

The ACJVS accepts MasterCard and Visa.

TUITION PAYMENT
All tuition and fees are due at the beginning of full-time training programs. However, all students participating in the Federal Financial Aid programs are allowed to attend classes while their aid is being finalized. Any money due to ACJVS will be credited to their account directly from the Federal Financial Aid programs before any moneys are disbursed to the student. To register, all students must pay a $15 tuition fee. Many students have course fees paid for by their employer. In cases where the students leave their employer and the employer discontinues tuition payment, the student is responsible for the payment of any remaining tuition. Full-time students must have an approved financial aid package or 10% of their course cost paid prior to the start of classes.

PAYMENT PLANS
Students in full-time courses may elect to pay for their course by payment plan. The financial aid specialist and the students design these plans. If a student chooses to pay for his/her course by payment plan, then the monthly payment must be paid according to the agreed upon schedule. Should a student not follow the schedule, they will not be permitted to attend class until the appropriate payment has been received. Missed classes will not be excused and will affect a student's attendance record.

FINANCIAL AID DISCLOSURE 
Students are responsible for disclosing all sources of financial aid they are receiving while attending school. Only US citizens or those eligible to attend school in the US can receive financial aid.

REFUND POLICY
No refund will be made for part-time courses if withdrawal is made after the first class meeting. There are no exceptions to this policy. Tuition for full-time programs paid in full will be refunded as follows:
80% if withdrawal occurs before the 6th day of class
60% if withdrawal occurs between days 6 and 10 of class
40% if withdrawal occurs between days 11 and 15 of class
0% if withdrawal occurs after 15 days of class.
The Federal Return of Title IV Aid worksheet will be used to determine refunds for students who receive any type of Title IV financial aid.

GRADING SCALE
All instructors use the following grading scale:
100-92% A
91-83% B
82-74% C
73-65% D 
Below 65% Failing
Please notify the financial aid specialist if your company requires you to be given a copy of your letter grade for tuition reimbursement.

SATISFACTORY ACADEMIC PROGRESS
a. Students must receive "C" or above in each core subject each term to continue their training. Further, their cumulative academic average must be "C" or above at the end of each grading period. This cumulative average is based on all academic activity from the first day of class through the end of each grade period.

b. Instructors will identify core and Non-Core subjects at the beginning of each grade period. This presentation will include how each subject will be weighted to determine overall grade for the term.

c. Grades and attendance are checked each time a financial aid payment arrives for a student. No disbursement is made to students 
who do not have at least 80% cumulative attendance and a cumulative grade average of "C".

d. If a student does not meet attendance and/or grade requirements, the Workforce Development Director will implement financial aid probation, academic probation, attendance probation, or dismiss the student from the school.

e. Probation: If a student fails to meet any of the requirements for satisfactory academic progress and/or attendance at the end of each term, consultation with the Workforce Development Director will be scheduled. The student may be placed on academic and or attendance probation for a 30-day period. At the end of the 30-day probationary period, the student must have a cumulative grade average of "C" and have 80% cumulative attendance. If a student fails to meet academic and attendance requirements, she/he will be subject to dismissal from the school. 

Note: Students are ineligible to receive Title IV (Pell Grants, etc) funds while on probation. 

f. The Workforce Development Director will inform the student of her/his probation/suspension status. The Workforce Development Director will notify the student of her/his dismissal from the school.

g. Appeal Procedure: A student may appeal a probation or suspension in writing. The appeal should be a letter addressed to the Workforce Development Director describing any undue hardship or circumstance, which may have caused the failure to meet the satisfactory academic progress, and/or attendance standards required by the school. A review board will be convened by the Workforce Development Director to consider the appeal. Appeals will be reviewed on an individual basis. Students will be notified of appeal decisions within two weeks.

h. Reinstatement: If a probation or suspension is successfully appealed, the student will be reinstated. Financial Aid eligibility will be reinstated at the end of the first grade period following the notice of suspension, provided the circumstances which caused the suspension/probation are corrected.

i. Repeating a class, Incompletes & Withdrawals: A student may repeat a program she/he failed or did not complete. Completion of the program must be within the maximum time frame (see Financial Aid Time Frame.) The repeat grade will replace any previous grade. 

A student receiving "I" or Incomplete grade will have one term to bring the incomplete up to a "C" average. 

j. Leaves of absences: A student interested in an extended leave from his/her class must apply in writing to the Workforce Development Adm.

AWARD OF DISTINCTION
Full-time students will graduate with distinction if they maintain 90% attendance at the end of each grade period throughout the program.

CAREER PASSPORTS / CERTIFICATES
Workforce Development Students completing programs at the school are awarded certificates as follows:
Ohio Department of Education - Division of Vocational Education
Students must:
a. Complete an approved vocational training program of at least 540 classroom hours/laboratory hours.
b. Attain a cumulative grade point average of 2.0 ("C") or better.
c. Maintain a cumulative attendance of at least 90%.

ACJVS - Workforce Development 
Students must:
a. Complete a training program offered by ACJVS.
b. Attain a cumulative grade average of 2.0 ("C") or above if the course is graded or a "P" if the course is Pass / Fail.
c. Maintain a cumulative attendance of at least 80%.

Career passports are awarded to full-time students during the March or September Graduation Ceremonies.

HIGH SCHOOL DIPLOMA OR G.E.D.
All students who wish to apply for any of the TitleIV funds must have a copy of their high school diploma, high school transcript, or G.E.D. scores on file with the school.

STUDENTS IN PHOTOGRAPHS
Unless the Workforce Development Office is notified in writing, students are assumed to have given permission to ACJVS to use pictures (still or video) of them, materials owned by them, and record their voice in connection therewith during school or at school related activities and to put the finished pictures or recording to any legitimate use without limitation, reservation, or compensation.

SCHOOL CLOSING
If conditions arise that cause Workforce Development classes to be canceled, students will be informed by an announcement on the following radio stations: WFUN, WREO, WDON, and WZOO. If the day school has been canceled due to inclement weather, Workforce Development classes in the evening are NOT automatically cancelled. Stay tuned to local radio stations regarding Adult Ed. cancellation notices.

SAFETY
Safety regulations are to be followed at all times according to the standards of the program in which you are enrolled. Failure to follow the prescribed safety requirements will result in removal from class and possible dismissal from the program. You are responsible for your own safety equipment. Appropriate attire for each program is required.

INSTRUCTOR AUTHORITY
The instructor for each class has complete authority within her/his area. The instructor is responsible for each student assigned to her/his class.

SUBSTANCE ABUSE
Students shall not possess, offer for sale, or be under the influence of any controlled substance or alcohol while on school property or at school sponsored events. Students who violate this policy shall be immediately dismissed from their program and referred to legal authorities.

PARKING
Students are to park their cars only in areas designated for students. Parking on grass and around the school drive is not permitted. No parking is permitted in the Fire Lane between buildings. Persons blocking the driveways will be asked to move their cars or their cars will be towed. Students are not permitted to return to their cars during class hours without permission from their instructor. Students are not allowed to leave the school premises and return without permission from their instructor. Failure to follow proper procedures will result in disciplinary action up to and including dismissal from school. Note: Building "A" students are not permitted to park in the first row until after 6:00pm.

CONDUCT AND BEHAVIOR
Conduct disruptive to the educational process or destructive to property will be cause for immediate dismissal from the school. A record of negative conduct and behavior will be maintained.

DISMISSAL
Violation of the school's rules, policies, and regulations may be cause for dismissal from the school and participation in ACJVS activities. A record of dismissal will be maintained.

TELEPHONES
ACJVS telephone lines are not available for student use. Instructors will inform you of the location of telephones for student use.

TOBACCO
Use of tobacco is not permitted in any building on campus. Instructors will inform you of designated, outdoor smoking areas.

BREAKS
All Workforce Development students are to take breaks in the Cafeteria or Workforce Development Break Room. No food or beverages are allowed in any other place on campus.

DRUG FREE SCHOOL / WORKPLACE POLICY
The following policy was established by ACJVS Board of Education in accordance with the Drug Free School/Workplace Act of 1988 (PL 100-590, 102 Stat. 4181, Section 5151-5160). This policy is intended to establish a drug free school/workplace. It is considered as a condition of employment and attendance.

a. The unlawful manufacture, distribution, dispensing, possession, or use of a controlled substance by any employee or student, which takes place in whole or in part in the school/workplace, is strictly prohibited.

b. An employee or student convicted of any Federal or State criminal drug statute within the school/workplace, must notify the school with in five (5) calendar days of the conviction.

c. Any school receiving Federal Funds, who is notified by an employee or student of school/workplace related drug conviction, must report it to the Contracting Agency within ten (10) calendar days of learning of such conviction. Any conviction not reported may result in the loss of Federal Funds.

d. The school must, within thirty (30) days after receiving notice of a conviction from an employee or student:
-take appropriate action against the employee or student up to and including termination.
-refer the employee or student to a Substance Abuse Rehabilitation & Intervention Program.

e. Any employee or student who fails to report a conviction is subject to immediate termination.

SEARCH FOR HANDICAPPED CHILDREN UNDERWAY
School districts across the State of Ohio are participating in an effort to identify, locate, and evaluate all handicapped children between the ages of 0 and 21. By handicap, we mean conditions such as hearing impairment, visual impairment, speech or language impairment, learning disabilities, behavioral, multiple or severe handicaps, mental retardation, and other health impairments.

Many handicapped children are not visible because they do not function in the mainstream of the community. Also, many unidentified handicapped children are preschoolers. Parents may not be aware that there are programs and services available.

If you know of a handicapped child, please contact the Special Needs Coordinator at ACJVS.

CRIME AWARENESS REPORT
The following information is provided and updated annually as directed by the U.S. Department of Education through Public Law 101-542, the "Criminal Awareness and Campus Security Act of 1990."

Campus Security Policies & Procedures
a. Reporting of Criminal Incidence - ACJVS strives to provide a safe and secure campus for all students and staff members. All students and staff members are encouraged to report any and all suspicious campus activity immediately upon witnessing the occurrence. All students and staff members should report any knowledge of a criminal or suspicious nature to the Workforce Development Director.

b. Security on Campus - The school is in operation from 8:00am to 10:15pm Monday through Thursday and 8:00am to 4:00pm on Friday. The building will be open to students and staff during these hours.

c. Campus Policy Regarding Illegal Substances - The school strictly forbids the possession, use or sale of any alcoholic beverages and/or illegal drugs on campus. Students and staff members should report any knowledge of such activities to the appropriate school personnel listed in Part "a" of this section. Any infraction is cause for immediate suspension and possible termination. When appropriate, such infractions will also be reported to legal authorities. Specific details of the school's drug policy are outlined in the campus drug free policy statement described annually to all staff members and students.

d.. Current Campus Crime Statistics - The following is a list of all criminal incidences reported on campus for the period July 1, 1999 to June 30, 2000.

ADDITIONAL POLICIES FOR FULL-TIME BUSINESS STUDENTS

HOSPITALIZATION / LEAVE OF ABSENCE
In the event of pregnancy leave or hospitalization, students may request that absence not be counted against their cumulative percentage of attendance. A maximum of fifty (50) hours may be excluded from the calculation. Requests for consideration under this section must be made in writing to the Workforce Development Department. Decisions will be made by a review panel appointed by the Workforce Development Director. Students must complete all work assigned during their absence prior to their request for special consideration.

EXTERNSHIP / EARLY PLACEMENT (Business)
Students may receive credit for work time related to their training provided the student:
a. Has completed at least three quarters (3/4) of their program.
b. Has ninety percent (90%) cumulative attendance.
c. Has at least a 2.0 ("C") cumulative grade average.

Student credit for work will be granted as follows:
a. Students will be given credit for each hour worked on a school day up to a maximum of three (3) hours daily.
b. Students must attend class the day they "work in lieu of school" credit is granted. This attendance must be between 6:30pm and 9:30pm.

ADDITIONAL POLICIES FOR FULL-TIME COSMETOLOGY STUDENTS
Adult Cosmetology is unique from other Workforce Development Programs offered by ACJVS. The uniqueness involves operation of a clinic, which provides services to patrons under the supervision of instructional staff. More than one instructor may be on the floor giving direction. As a result, there must be a common understanding of clinic policies.

LANGUAGE
No profanity or gossip is allowed. Additionally, signaling assistance from an instructor is to be done in a professional manner without disturbing patrons.

DRESS
Students must be dressed in their uniform while performing patron services. Clothes should be clean and neatly pressed. Shoes should be of conservative styling, polished, and conform to the safety standards of the Cosmetology Clinic. Students not dressed appropriately will not receive hours for the day.

ATTENDANCE
a. Students are expected to arrive on time to work the clinic. Tardiness is not tolerated. Tardy students will lose time in one half (1/2) hour increments.

b. Pregnancy leave is granted with documentation from a physician. A physician must grant the student permission for re-entry. Re-entry dates and class assignments will be developed by the school administration with input from the instructional staff.

c. Students are not permitted to leave the clinic until dismissed by their instructor.

d. The instructional staff with the interest of the patrons in mind will grant break times. Students must sign in and out in order to receive hours.

e. Students are expected to complete the program within the allotted time period. Students not completing the program on time will be required to pay an hourly rate for hours accumulated after their scheduled graduation date. Additional hours may be purchased in one week, ,twenty-six (26) hour increments and are payable in advance only.

FOOD AND BEVERAGE CONSUMPTION
Consumption of food and/or beverages in the clinic is strictly prohibited.

PATRONS
Patrons will be assigned to students on a rotating basis. As a result, all students have the same opportunity to experience a diversity of instructional experiences. No requests will be granted. Please note: All conversations with patrons should be professional. (No profanity or gossip.)

INSTRUCTOR'S OFFICE
Students are not permitted in the instructor's office unless accompanied by an instructor.

TELEPHONE
The telephone in the Cosmetology area is not for personal use. Students must use the pay telephone in the Building "B" Cafeteria. 

COSMETOLOGY KITS
Students are not permitted to remove their kits from campus until they have accumulated 300 hours. The kit remains property of the school until that time. Kits left at the school after graduations are disassembled after six (6) months.

WITHDRAWAL / RE-ENTRY
a. Students may officially withdraw from the program by notifying their instructor and completing a withdrawal form in the Workforce Development Office. The date the withdrawal form is completed is considered the official withdrawal date. Days missed prior to the official withdrawal date are considered absences.

b. Students requesting re-entry into the program will be admitted at a point identified by the school. Typically, this will be when the current class has accumulated the same amount of hours as the re-entering student.

c. Students seeking re-entry will be required to pay a $100 re-entry fee. This amount is in addition to any other charges incurred. The fee is non-refundable and not covered by financial aid.
